Release and Renew Workshop Participant Consent & Commitment Agreement
Please read carefully, then sign and date to indicate your consent and agreement with these commitments.
Everything stays within the group. I agree to respect the confidentiality of all people in this training. I agree to keep anything that is said or done in this training confidential. The exception to this confidentiality is in the case of a disclosure of intention of suicide or harm to another.
I am participating of my own choice and not as a result of coercion, pressure or to satisfy anyone other than myself. I understand that I have the choice to participate, or decline, any exercise or sharing. I understand that this workshop is an educational forum for support and connection, and not a substitute for psychotherapy.
I assume the risk, by this consent, of any accident or illness during the workshop and hereby release Sheron Alter and Mirabai Galashan and the owners of the location from any liability.
I understand that in addition to the benefit, there is always the risk of emotional and/or medical contingencies in such a group experience. I authorize the responsible persons to take any reasonable steps on my behalf in the case of an accident, injury or illness, including, but not limited to, emergency first aid, doctor, nurse and/or ambulance services, etc. I agree to be liable for the cost of any such action taken on my behalf, and hereby release the facilitators and hosts from all liability ensuing. I hereby agree to the above.
